In the space provided below, please list any University/efactory equipment, computers, materials, furniture, etc. that you are requesting to borrow as well as the proposed loan period. Items found in my possession or other off-campus locations that are not declared on this statement will be assumed stolen property.
Please fill out this form for each TYPE of equipment, furniture, etc. that you are borrowing. e.g. If you are borrowing two chairs that are the same, you can complete ONE form for BOTH chairs. If you are borrowing one chair and one desk, you will need to complete TWO forms, one for EACH item.
